The cost of your group medical plan
will vary based on the demographics of your group and your
health conditions. Other important factors are plan design
including the level of copays and out-of-pocket expenses, and
whether or not you plan provides of out-of-network benefits
Our job is to help you sort through these factors and help you
make the best decision for your employees.
To provide us with
the necessary information, the employer must complete the
Group Information Sheet
(PDF). You can
print it out, compete it and fax it to us at 821-6488. Or
complete it and submit it on-line. Please let us know how
many Universal Health Appraisal Questionnaires we should expect
to receive.
The
Universal Health Appraisal Questionnaire (PDF)
must be completed by each
participating employee and employees who will meet your
eligibility requirements by the effective date of the new plan.
It is important to maintain the confidentiality of the employees
health information. They can print out the form, complete
it and fax it to us at 821-6488. Or they can complete the
form, place it in a sealed envelope and turn it in to the HR
Department or the office manager. We can then pickup the
Questionnaires or you can mail them to us.
It is very important that the employees answer each question
carefully. Any yes answers must have an explanation.
The forms must be signed and dated. We regret the
inconvenience that all of this paper work requires, but it is
the only way to receive accurate proposal rates from other
plans.
